As a Driver I hereby agree to the following:

I will maintain full coverage insurance on the vehicle used to transport for Franchisee.
I will confirm with my insurance agent that my coverage is valid when contracting my services.
I will use due care and caution in the operation of my  vehicle and will strictly comply with all safe driving rules of the road, including all speed limits, posted directional signs and parking regulations. Under no circumstances will I operate my  vehicle while under the influence of drugs or alcohol or when my physical or mental condition may be otherwise impaired. 
The vehicle I use for  services will:   Be maintained in good condition and repair.  Comply with all rules and regulations governing safe and unlawful operation.   
I will immediately notify you if I am involved in any accidents or receive any subsequent citations during the course of my employment.
I acknowledge the need for utmost safety and due care in the operation of my  vehicle and in the conduct of services. 
I agree to only operate the vehicle approved by Franchisee and will not drive a substitute vehicle without carrier’s prior approval. 
 I agree to maintain my automobile insurance (including coverage for Bodily Injury, Property Damage and Personal Liability) AT ALL TIMES while using my vehicle  and will inform Franchisee if my insurance coverage is changed, canceled or not renewed. 
 I understand that I may be personally responsible for the insurance deductible if I am involved in an accident of any sort involving my vehicle and the travel trailer in delivery or picking up that causes injury or damage to another person and/or their property and/or the trailer I'm towing. I am also aware that Franchisee’s insurance DOES NOT cover my vehicle for comprehensive or collision coverage. 
I understand any damage caused be me to the trailer while in my possession would be my responsibility to fix and Franchisee can withhold pay to help cover damages. 
I understand the liability for any tire damage and/or blow outs shall be determined by a third-party tire shop. I understand any such tire problems that are attributed to me by them will be my responsibility to replace.
I understand that I am responsible for having my own tools
I agree to complete all forms via website (Setup, Pickup, Delivery Time Log, Tote, Etc) as directed and notify Franchisee immediately of any issues, damages, etc
